If you would like accommodation or support from LEAP firstly, make sure you meet our referral criteria. If you meet the criteria then please complete our referral application form.
Referral Application Form - by either filling in the referral form online or by downloading a copy and returning it to us either in person, by post or by fax (contact details can be found on the contact us page).
You can either self refer or ask an agency to help you.
Self referral – If you are not referred by an agency you will need to provide the names of 2 people who can give you a reference, this can be anyone who knows you well, such as your college tutor, an employer or a previous landlord.
Agency referral - If you are involved with an agency such as Connexions, ISIS, Youth Offending, Barnardos, Housing, etc you can ask your key worker to make a referral with you. Please note that if you are a care leaver the agency making your referral must either be your Social Worker or your leaving care worker.
Help - If you are not sure what to do or you would like some help completing the form you can arrange to come to the LEAP office where a member of staff will be happy to help you.
Processing Application Forms - Once we receive your completed form you will be contacted and an interview will be arranged. The interview will take about 1 hour to complete. Your application and interview will then be discussed at our team referral meeting where a decision will be made. We will contact you after the meeting and let you know the result.
Right to Appeal - If you are not happy with the decision that we make you have the right to appeal, you can do so by completing an appeal form.
